PHOTO PURCHASE INSTRUCTIONS

As part of the Bellaire High School Volleyball Booster Club fund raiser, we are selling action photographs of your children.  Please follow the simple instruction below.

1. After you decide which photos of your children you want to purchase, you will fill out a simplified order sheet that includes the following information:

Please make sure you include your name, street address, and zip code for mail orders.

You can download an electronic worksheet by clicking here.  When the “Open,” “Save,” “Cancel,” or “More Info” appears choose Save and download to you hard drive.

2. Because all work is done by volunteers, we ask that you help by making minimum order of $25 or making a combined order through a team contact such as a team mom. 

3. After filling out your order form, (including contact information - phone numbers) you should email it to cmfebles@earthlink.net. 

4. Your order will be reviewed and you will be notified if there are any problems with any of the photos you selected.  Once your order is processed, arrangement will be made for payment and delivery, e.g. postal services, or other.  Note all checks should be written to BHS Volleyball Booster Club.

5. Pricing Schedule is shown below:

• 4X6                       $ 3.00
• 5X7                       $ 5.00
• 8X10                     $10.00
• Poster 16x20       $30.00  *
• Electronic File      $25.00
• Shipping               $2.00 - $5.00 depending on quantity

Note:  All photographs are protected by copyright law and may not be reproduced anywhere, for any reason, in print, on the web or any other medium.  Do not copy or scan them to another computer, transmit, publish, reproduce, alter or use them in any way without the written approval of FeCon, Inc.  By purchasing the photographs you agree to these terms.  If you would like to publish the photographs, e.g. in a yearbook, please contact me for special pricing for the electronic files that are sent directly to the publisher.
